Bridging the ENT training gap: Evaluating an intermediate elective skills course for junior ENT trainees

Abstract

Introduction: ENT is a hands-on specialty, yet opportunities for structured training in elective otolaryngology procedures remain limited for junior trainees. To address this, we developed an innovative, simulation-based, elective ENT surgical skills course. This has been delivered annually since 2021 and to our knowledge, is the only UK course specifically designed with this focus. This study evaluates outcomes from cohorts in 2021 and 2024.

Methods: The course was developed using a combination of consultant-led lectures and simulation-based practical stations for 6 common, junior, elective ENT operations. Participants completed pre- and post-course questionnaires capturing demographics, baseline surgical experience, confidence ratings (1–5 Likert scale) and qualitative course feedback. Descriptive analysis was performed using Microsoft excel.

Results: 37 trainees completed pre-course surveys. 86% of attendees were core surgical trainees or clinical fellows, drawn from 12 UK deaneries. Pre-course confidence was low across all skills, with average ratings ranging from 1.24 – 2.32. Post-course data was available for 33 trainees. Confidence improved in all domains: grommet insertion (pre 2.2, post 4.1), tonsillectomy (2.3, 4.0), adenoidectomy (1.7, 3.9), panendoscopy (2.1, 3.7), Microlaryngoscopy (1.9, 3.7), and FESS (1.6, 3.4). Qualitative feedback was highly positive: 100% agreed the course was enjoyable and of educational value and feedback emphasised the value of hands-on simulation and appropriate course level.

Conclusions: This ENT skills course significantly enhanced trainee confidence across core procedures and was consistently well received. Outcomes suggest such structured, simulation-based training fills an important educational gap and we provide a scalable model which warrants regional or national expansion.
